Summary: The Clinical Pharmacist Independent Prescriber role is focused on enhancing patient care within a primary care setting by optimizing medication management and improving outcomes. The pharmacist will work collaboratively with a multi-disciplinary team, conducting medication reviews and providing expert advice, particularly for patients with complex and long-term conditions. Key Responsibilities:

  • Independently prescribe acute and repeat medications and lead medication reviews.
  • Conduct clinical assessments and treat patients, particularly those with chronic conditions or polypharmacy.
  • Provide specialist expertise in medicines optimisation and collaborate with healthcare professionals.
  • Support patients with complex medication regimens and manage high-risk drugs.
  • Drive quality improvement initiatives and handle prescription queries.
