Summary: The Children's Continuing Care Case Manager for CAMHS/CALDS is responsible for providing integrated services for children meeting continuing care criteria, particularly in mental health and learning disabilities. This role involves case management, monitoring care delivery, and supporting Care Coordinators in developing care packages. The position is a fixed-term contract covering maternity leave for 12 months. The ability to speak Welsh is desirable, and the role supports flexible working arrangements.
Key Responsibilities:
- Provide case management for a caseload of patients/service users.
- Develop and implement appropriate packages of care.
- Monitor clinical compliance and implement a review program.
- Chair multi-disciplinary team meetings to ensure robust decision-making.
- Ensure lawful commissioning decisions align with national priorities.
- Support Care Coordinators in new referrals for Children's Continuing Care.
- Quality assure Children's Continuing Care applications.
- Present cases at the Quality Assurance/Peer Review Panel.
- Report on provider performance and follow up on action plans.
- Conduct reviews to ensure adherence to contractual arrangements in clinical care delivery.

- Experience in case management within health services.
- Knowledge of Children's Continuing Care criteria and frameworks.
