Position: Qualified Social Worker
Location: Cheshire East - Adult Community Team
Pay Rate: 35 per hour
Contract Type: Temporary
About the Role
We are seeking an experienced and capable Qualified Social Worker to join the Adult Community Team in Cheshire East. This role is pivotal in supporting the East teams with discharges from out-of-area hospitals, including Wythenshawe, MRI, and Trafford.
While based within the community team, you will focus on ensuring timely and effective hospital discharges while supporting individuals to transition smoothly back into their communities. This is a fast-paced role, requiring strong assessment skills and decision-making under pressure.
Key Responsibilities:
Complete assessments to determine needs and support plans.
Conduct Mental Capacity Act assessments and facilitate best interest meetings.
Coordinate hospital discharges and work closely with external partners.
Provide short-term interventions to promote independence and reduce the need for long-term care.
Manage a varied caseload efficiently and meet deadlines.
Requirements:
Qualified Social Worker (Degree or equivalent in Social Work).
Registration with Social Work England.
Proven experience in adult social care, particularly in hospital discharge or community teams.
Strong knowledge of the Mental Capacity Act and experience conducting best interest meetings.
Ability to work under pressure and at a fast pace.
