Position: Qualified Social Worker (QSW)
Team: CAD Team (Front Door)
Location: Bury
Pay Rate: 30 per hour
Are you an experienced and proactive Qualified Social Worker looking to join a fast-paced and impactful team? The CAD Team in Bury serves as the front door to adult care services, playing a critical role in triaging and addressing a variety of referrals.
Key Responsibilities
As a member of the CAD Team, you will:
Triage Referrals: RAG rate all referrals to Bury Adult Care Services.
Service User Engagement: Contact service users meeting CAD Triage criteria to assess safety and wellbeing.
Multi-Disciplinary Liaison: Collaborate with professionals such as CMHT, OPCMHT, Police, Hospitals, and GPs to ensure effective case handling.
Resource Coordination: Identify and connect users with relevant services, including Food Banks, BAME Groups, and Men/Women/LGBT+ Support Groups.
Initial Needs Assessments: Conduct assessments to facilitate housing or other required services.
Home Visits: Perform visits when necessary to address immediate needs or risks.
Risk Management: Recognize and take appropriate action when risks are identified.
Enquiry Support: Support Enquiry Officers by managing concerns passed to CAD Triage.
EDT Notifications: Perform follow-ups or signpost to appropriate teams.
Case Management: Handle SF1 forms that do not meet Safeguarding Team criteria and require CAD Triage oversight.
What We're Looking For
A qualified social worker registered with Social Work England.
Experience in adult care services, front door services, or triage roles.
Strong decision-making, communication, and risk assessment skills.
Ability to work collaboratively in a multi-disciplinary environment.
