? Job Opportunity: Qualified Social Worker - Adults Locality Team ?
? Location: Civic Centre, Bromley BR1 3UH
? Pay Rate: Up to 32 per hour
Role Overview
Join the Hayes and Five Elms Locality Team in Bromley as an Adult Social Worker, supporting younger adults with physical disabilities, sensory impairments, and older people to live independently. The role involves conducting assessments, managing care plans, and working with partner agencies to deliver high-quality care.
Key Responsibilities
Conduct Care Act 2014 assessments and facilitate hospital discharges.
Manage annual reviews of support plans, direct payments, and extra care housing support.
Support young people transitioning from Children's Services to Adult Services.
Undertake Safeguarding (s42) enquiries, Mental Capacity Assessments, and Best Interest decisions.
Work as a duty worker on a rota basis.
Build and maintain strong relationships with partner agencies and multidisciplinary teams.

Hybrid Working: Flexible arrangements, with 2 days per week in the office and face-to-face visits to families.
Travel Expenses: Mileage reimbursement for travel from the workplace to visits (excluding home-to-work travel). Work Oyster cards are available for public transport.

Experience and Skills Required
Essential:
Proven experience in care management and Safeguarding (s42).
Knowledge of the Care Act 2014, Mental Capacity Act, and transition planning.
Strong communication and negotiation skills.
Ability to work collaboratively with multidisciplinary teams and partner agencies.
Desirable:
Previous experience supporting younger adults with disabilities or older people.
Knowledge of Bromley's local services and resources.
Working Arrangements
Hours: Full-time, 36 hours per week.
Office Attendance: Minimum 2 days per week.
Location: Civic Centre, Bromley BR1 3UH.
Expenses: Mileage and work Oyster cards for travel.
