Summary: We are looking for a dedicated Band 5 Physiotherapist to join our Intermediate Care Community team in Bridgewater. The role involves providing high-quality physiotherapy support to patients transitioning between hospital and home, working in both community settings and bed-based units. The successful candidate will collaborate with a multi-disciplinary team to facilitate early discharges and prevent unnecessary hospital admissions. This position offers flexible working hours and the opportunity to develop experience in community-based physiotherapy.

Key Responsibilities:

• Assess, plan, and deliver physiotherapy interventions to patients in an intermediate care setting.
• Support early discharge from hospitals and prevent unnecessary hospital admissions.
• Work collaboratively within a multi-disciplinary team, including nurses, occupational therapists, and social care professionals.
• Provide rehabilitation and mobility support tailored to individual patient needs.

Key Skills:
