Bradford is seeking a Qualified Social Worker (QSW) to join their Community Mental Health Team. This is an exciting opportunity to work in an integrated setting, delivering high-quality services to individuals with mental health needs.

Key Responsibilities:

Fulfill Local Authority duties under the Care Act, ensuring compliance with statutory requirements.
Undertake care coordination functions within an integrated mental health team.
Provide assessments, planning, and interventions to support individuals and their families.
Collaborate with multi-disciplinary professionals to deliver effective and person-centered care.

Requirements:

Qualified Social Worker, registered with Social Work England.
Experience in mental health social work and care coordination.
Strong understanding of the Care Act and Local Authority statutory functions.
Excellent communication and organizational skills to thrive in a collaborative team environment.
