Summary: The Senior Practitioner - Discharge to Assess Community (ABI) role at Birmingham City Council involves managing complex cases related to Acquired Brain Injury. Key Responsibilities:
- Conduct face-to-face Care Act assessments for highly complex cases.
- Participate actively in the CHC process and attend MDT meetings, primarily via Teams with occasional face-to-face attendance.
- Supervise and support social care workers within your cluster/team, fostering a culture of continuous improvement and high-quality practice.
- Lead regular supervision sessions, manage caseload complexities, and ensure timely and efficient service delivery.
- Undertake case file audits to promote shared learning and improvements across the team.
- Drive team performance, community development, and partnership working through a One Team approach.
- Travel will be reimbursed for journeys beyond the first trip from home.
Key Skills:
- Eligibility for registration with Social Work England.
- Experience in the Continuing Health Care (CHC) process.
- Understanding of Acquired Brain Injury (ABI) and its impact on individuals.
- Ability to challenge health professionals appropriately and support social workers in doing so.
- Strong leadership and supervisory skills with the ability to manage a team in a high support, high challenge environment.
