Job Advert: Qualified Social Workers - Hospital Social Work Team
Location: Barnsley
Pay Rate: 35 per hour
Contract Type: Hybrid
We are seeking two experienced Qualified Social Workers to join the Hospital Social Work Team in Barnsley. This is an excellent opportunity for skilled professionals with a strong understanding of hospital discharge processes and a commitment to delivering person-centred care.
Key Responsibilities
Support hospital discharges using the Discharge to Assess process, ensuring individuals receive appropriate assessments and support.
Collaborate closely with health colleagues in acute and intermediate care settings to achieve the best outcomes for individuals.
Complete Care Act and Mental Capacity Assessments, and contribute to best interest decision-making.
Undertake safeguarding investigations, adhering to safeguarding procedures and ensuring timely interventions.
Lead on complex discharge planning, promoting a strengths-
based approach to care and ensuring individuals are placed on the correct pathways.
Requirements
Professional Qualification: Qualified Social Worker status with at least 12 months post-qualification experience.
Knowledge:
Strong understanding of the Care Act 2014, Mental Capacity Act 2005, and Human Rights Act 1998.
Familiarity with safeguarding procedures and processes.
Skills:
Excellent ability to collaborate with multi-disciplinary teams.
Strong assessment and decision-making skills, particularly in complex cases.
Flexibility:
Ability to respond to urgent requests across the borough at short notice.
Willingness to work in a hybrid model, combining home working, office-based, and hospital-based tasks.
