Summary: As a Band 6 Occupational Therapist at Guys and St Thomas NHS Trust, you will deliver comprehensive therapy assessments and interventions for patients in community settings. The role requires collaboration with healthcare professionals to ensure high standards of care and effective discharge planning. A minimum of 6 months NHS experience is mandatory for this position. You will work autonomously in various environments, including urgent response situations.
Key Responsibilities:
- Conduct patient-facing therapy assessments, risk assessments, and clinical reasoning to develop tailored intervention plans
- Set therapy goals and plan interventions to support patient recovery and independence
- Plan and coordinate safe and effective discharges
- Prescribe, review, and monitor equipment to meet patients' needs
- Work autonomously and effectively in a community setting, including urgent and rapid response environments
- Utilize EPIC and other electronic health record systems for documentation and care planning
- Comply with the standards set by the Health & Care Professions Council (HCPC) and other relevant professional bodies
Key Skills:
- Professional registration with the Health & Care Professions Council (HCPC)
- Experience in urgent/rapid response and community-based settings
- Competence in risk assessments, clinical reasoning, and lone working
