Summary: The role of Band 6 Community Psychiatric Nurse involves joining the Community Mental Health Recovery Service in Frimley to support individuals with mental health needs. The position requires conducting comprehensive assessments, developing care plans, and collaborating with multidisciplinary teams. Key Responsibilities:
- Undertake comprehensive initial assessments of individuals accessing mental health services
- Support the management and allocation of waiting lists
- Conduct thorough mental health assessments, formulating both mental health and social needs of service users
- Make evidence-based recommendations for onward care, including external referrals
- Present case formulations and care recommendations within MDT meetings
- Develop person-centered care plans with robust risk assessment and management strategies
- Liaise effectively with external agencies and key stakeholders to ensure high-quality, coordinated care
- Engage with service users' support networks, including carers and family members
- Deliver brief interventions such as emotional coping skills, trauma stabilization, and psychoeducation
Key Skills:
- Registered Mental Health Nurse (RMN) with valid NMC registration
- Relevant NHS experience in a community mental health setting
- Excellent verbal and written communication skills
- Proven ability to conduct comprehensive mental health assessments
- Strong care planning and risk management capabilities
- Experience working collaboratively within multidisciplinary teams
- Desirable: Experience using SystmOne clinical system
- Desirable: Skills in delivering brief psychological interventions (training can be provided)
