Summary: The Band 6 Community Physiotherapist role at Hertfordshire Community NHS Trust involves providing assessment, diagnosis, and treatment to patients with diverse needs within the Integrated Care Team. Key Responsibilities:
- Provide physiotherapy to patients within the Integrated ICT and Community Nursing Teams.
- Assess and treat vulnerable patients with complex needs and those in crisis.
- Act as a patient advocate throughout their rehabilitation program.
- Communicate complex patient-related information effectively.
- Collaborate with multidisciplinary team members across health and social care sectors.
- Utilize verbal and non-verbal communication skills to support patient rehabilitation.
- Identify and address barriers to effective communication.
- Maintain accurate documentation in line with legal requirements.
- Advise on professional and clinical issues.
- Communicate effectively with relatives and carers regarding patient care.
Key Skills:
- Physiotherapy Degree.
- Registration with Health and Care Professions Council (HCPC).
- Strong post-registration experience.
- Experience working as an autonomous practitioner.
- Excellent oral and written communication skills.
- Ability to communicate where there may be barriers to understanding.
- Knowledge of clinical governance and its implications.
- Strong organizational and time management skills.
- IT skills, including knowledge of word processing and internet.
- Full, valid UK driving license and access to a car for business purposes.
