Summary: The Band 6 Community Nurse position offers an opportunity for an experienced individual to join a city-wide service within the Learning Disabilities Service. The role involves working collaboratively within a multi-disciplinary team to provide support and guidance to individuals with learning disabilities and their carers. The successful candidate will manage a clinical caseload under the supervision of Senior Community Nurses, focusing on improving health outcomes. This position is part of Birmingham Community Healthcare NHS Trust, which is dedicated to delivering integrated, personalized care.

Key Responsibilities:

  • Manage and co-ordinate a caseload under the supervision of the Senior Community Nurse.
  • Provide support and guidance to children and adults with learning disabilities and their carers.
  • Focus on individual health outcomes and assist in accessing health services.
  • Work within the framework of a multi-disciplinary team.
  • Participate in on-call duties as required.

Key Skills:

  • Registered Nurse: Learning Disabilities or Mental Health.
  • Significant post-registration experience working with multi-disciplinary teams.
  • Knowledge of assessment processes and legislation related to learning disabilities.
  • Good IT skills and effective communication skills, both written and verbal.
  • Ability to undertake and understand audit tools.
  • Confident and assertive personal qualities.
  • Car driver with full license and access to a vehicle.
