Summary: The Band 6 Community Mental Health Nurse is responsible for managing a case-load while actively participating in the weekly Duty system. Key tasks include conducting health and social care needs assessments, risk assessments, and developing care plans, as well as monitoring patients' mental states and coordinating medical reviews. The role also involves completing referrals to care pathways and providing support to identified carers.
Key Responsibilities:
- Manage a case-load within the team.
- Participate in the weekly Duty system.
- Complete Health and Social Care Needs assessments and Risk Assessments.
- Conduct Dialog+ interventions and develop Safety Plans and Care Plans.
- Monitor patients' mental states.
- Arrange and attend medical reviews with the team of Doctors.
- Complete referrals to relevant care pathways.
- Offer support to identified carers and provide signposting for additional support.
Key Skills:
- Experience in mental health nursing.
