Summary: The role involves providing early assessment and intervention as a Band 6 or Band 7 Occupational Therapist and Physiotherapist in the Same Day Emergency Care Unit. The position is part of the Frailty Therapy Team, focusing on delivering integrated therapy to facilitate safe patient discharges. The practitioner will work autonomously and collaboratively within a multidisciplinary team to address various deficits impacting patient functionality and safety at home. The emphasis is on a proactive rehabilitation approach with a 'Home First' philosophy.

Key Responsibilities:

• Provide therapy support to the Same Day Emergency Care Unit.
• Conduct comprehensive, generic, and holistic assessments of patients.
• Deliver specialist assessment and intervention for individuals with physical, cognitive, social, or perceptual deficits.
• Work independently as an autonomous practitioner and collaboratively with the multidisciplinary team.
