Key Responsibilities:
- Assess the needs of individuals with complex needs, considering risk levels and strengths.
- Provide professional guidance to clients and their carers for informed decision-making regarding care services.
- Produce person-centred support plans that promote independence and well-being.
- Conduct accurate and timely assessments of service users’ support needs.
- Maintain comprehensive case records in line with the Open Files Policy.
- Work collaboratively in a multi-disciplinary team and negotiate with other agencies on behalf of service users.
- Communicate effectively with service users, carers, and professionals in writing and verbally.
- Manage crises proactively and preventively.
Key Skills:
- GCSE Grade (or equivalent) in English Language and Mathematics.
- A Social Work Qualification (CQSW/Diploma in Social Work, Degree in Social Work) or a recognised Occupational Therapy qualification.
- Social Work England (SWE) Number.
- Advanced skills in Microsoft Office (Word, Excel, Outlook) and social media.
- At least 2 years' experience in a Local Authority, Health, or voluntary sector setting.
- Knowledge of relevant legislation and principles of personalisation, independence, and choice.
- Experience working with older or physically disabled individuals and knowledge of service provisions.
- Ability to work independently, prioritise workload, and meet deadlines.
- Strong communication skills, both written and verbal.
- Ability to keep comprehensive case records.
- Ability to communicate in a language other than English (desirable).

Qualification – Essential
- GCSE Grade (or equivalent) in English Language and Mathematics.
- A Social Work Qualification (CQSW/Diploma in Social Work, Degree in Social Work) or a recognised Occupational Therapy qualification, or a relevant community Nursing qualification.

- Advanced – Microsoft Office (Word, Excel, Outlook), social media.

Successful candidates will have:
- At least 2 years' experience in a Local Authority, Health or voluntary sector setting.
- Knowledge of relevant legislation and a fundamental understanding of, and commitment to, the principles of personalisation, independence and choice.
- Experience of working with older or physically disabled people and knowledge of service provisions for these groups.
- Experience of working in a multi-disciplinary way.
- Experience in assessment work.
- Experience of and commitment to teamwork.
- An understanding or experience of budgets and financial controls.
- Ability to carry out accurate and timely assessments and promote self-directed care.
- An ability to communicate clearly in writing, verbally and through reports with service users, carers and other professionals.

- Ability to negotiate on behalf of the Authority and/or the service user, with other agencies.
- Ability to work proactively, to pre-empt / and or prevent and manage a crisis.
- Ability to work as a team member in a multi-disciplinary service.
- Ability to keep comprehensive case records, which can be shared with service users in line with the Open Files Policy.
- Ability to undertake a comprehensive assessment of service users’ support needs.
- Ability to communicate professionally in a language other than English (desirable)
