Summary: Tripod Partners is seeking an experienced Advanced Community Practitioner to join a high-performing team focused on delivering expert clinical care and case management in the community. The role involves working with multidisciplinary teams to reduce hospital admissions and support proactive care for individuals with frailty. Key responsibilities include patient assessments, treatment management, and collaboration with various health services. The position requires a strong background in community frailty or acute care, along with relevant qualifications.

Key Responsibilities:

  • Identify patients at high risk of admission and deliver early, proactive intervention.
  • Carry out holistic assessments and develop personalised or advanced care plans.
  • Liaise with GPs, social care, therapy colleagues, and mental health services.
  • Work as a Non-Medical Prescriber (NMP) to initiate and manage treatment plans.
  • Support patients with long-term conditions and complex health needs.
  • Contribute to Virtual Ward and discharge pathways.
  • Maintain clinical records, contribute to audits, and uphold service quality standards.
  • Provide clinical supervision, mentoring, and peer support.

Key Skills:

  • Registered with NMC or HCPC.
  • NMP qualification.
  • Strong experience in community frailty or acute care.
  • Proven ability to work autonomously and as part of an integrated team.
  • UK driving licence and access to a vehicle during the working day.
