Key Responsibilities:
- Undertake comprehensive health assessments for adults and paediatric patients.
- Assess, diagnose, plan, implement and evaluate treatment for undiagnosed conditions.
- Perform clinical examinations and interpret physiological and psychological findings.
- Manage a clinical caseload with autonomy, making timely decisions within governance frameworks.
- Contribute to patient safety, quality improvement, and effective multidisciplinary working.
- Maintain professional competence and development in line with regulatory requirements.
Key Skills:
- Current professional registration with NMC or HCPC.
- Experience in a GP practice setting (2+ years preferred).
- Advanced clinical assessment and decision-making skills.
- Non-Medical Prescriber (V300) qualification.
- Level 7 Clinical Examination and Diagnostics qualification.
- Ability to practice autonomously and manage complex clinical presentations.

Job Introduction
Are you an Advanced Clinical Practitioner? Primary Care 24 (PC24) is seeking a highly skilled and motivated ACP to join our multidisciplinary primary care team. You will work within GP practices across Merseyside, delivering high-quality, patient-centred care and contributing to improved health outcomes for the local population. You will practice withautonomy, using advanced clinical skills to assess, diagnose, treat and manage patients both face-to-face and via telephone consultations. This role offers variety, professionalchallenge and the opportunity to work within a pragmatic and forward-thinking primary care environment.
Salary: £62,682.00 - £64,371.00 per annum (pro rata)
Location: Various including 15 Sefton road, Litherland , Maghull, Thornton Health centre & Crosby/Crossways
Shifts: We can package any of the below to suit your availability -together
Monday 10.30am-6.30pm (15 Sefton Rd)
Wednesday 8am-4pm (Crosby/Crossways)
Friday 8am-4pm (Litherland)
What will you be doing
- Undertake comprehensive health assessments for adults and paediatric patients, applying advanced clinical judgement and holistic care planning.
- Assess, diagnose, plan, implement and evaluate treatment and interventions for undifferentiated and undiagnosed conditions.
- Perform clinical examinations and interpret findings from both physiological and psychological perspectives.
- Manage a clinical caseload with a high degree of .autonomy, making safe and timely decisions within governance frameworksagreed
- Contribute to a culture of patient safety, quality improvement and effective multidisciplinary working.
- Maintain professional .competence and development in line with regulatory requirements, contributing positively as a senior clinician within the practice team
This role is designed for ACPs who they are working autonomously in a GP practice setting. The post holder will have access to GP clinical supervision and clear escalation pathways, with . PC24 promotes a pragmatic and .feelconfidentsupport focused on safedecision-making, governance and clinical case discussioninclusive working environment, where clinicians are encouraged to seek advice when required and professional accountability isshared within established systems of care
What do I need
Essential
- Current professional registration with the Nursing and midwifery Council (NMC) or Health and Care Professional Council (HCPC) with no conditions or restrictions.
- Experience working in a GP practice setting (2+ years preferred).
- Advanced clinical assessment, examination and making skills.decision-
- Non-Medical Prescriber (V300) qualification.
- Level 7 Clinical Examination and Diagnostics qualification.
- Ability to practise autonomously and manage complex clinical presentations.
Desirable
- MSc in Advanced Clinical Practice (or working towards completion).
- Evidence of leadership, service improvement or teaching contributions.
