Summary: The Adult's Social Worker role in Liverpool involves managing a diverse caseload within the framework of Adult Social Care, ensuring a strength-based approach to assessments and decision-making. Key Responsibilities:

  • Manage a varied caseload with appropriate supervision and support.
  • Ensure a strength-based approach in all assessments and conversations.
  • Conduct risk assessments and make informed decisions regarding caseloads.
  • Represent the department in multi-disciplinary forums and chair meetings as needed.
  • Work directly with individuals, families, and carers in line with safeguarding policies.
  • Act as a care navigator for individuals with eligible social care needs.
  • Plan responses to meet individual needs in collaboration with relevant stakeholders.
  • Monitor and review support plans and service standards to identify unmet needs.
  • Collaborate with Social Workers on tasks and areas of work.
  • Conduct mental capacity and best interest assessments when necessary.
  • Participate in integrated teams and complete relevant health care checklists.
  • Model inclusive practices and challenge discriminatory issues.
  • Engage in professional supervision and continuous development.
  • Ensure timely and high-quality report submissions.
  • Authorize and record funding for care packages.
  • Maintain compliance with data protection laws regarding client information.
  • Promote equal opportunities and non-discriminatory practices.
  • Complete safeguarding investigations and MHA tribunals as appropriate.
  • Implement s.117 aftercare policy in practice.
