Key Responsibilities:
- Evaluating the healthcare needs of patients to develop personalized care plans
- Providing complex care, including wound management, administering medications, and managing chronic conditions

- Assessing the need for at-home medical equipment, such as mobility aids or specialist beds
- Providing follow-up care for patients recently discharged from the hospital to ensure a smooth transition and prevent readmission
