Job OverviewThe role involves care co-ordination of clients within the CMHT/OP service to provide high quality care.Following a redesign of services, we are in an exciting position to be able to recruit a Senior Community Mental Health Nurses for our Older Adult Services in South East Essex - primarily your caseload would be working either within Castlepoint or Rochford Locality . This is a supportive, well-resourced multi-disciplinary team with Support Workers, Physical Health Practitioners, Qualified Nursing and Medical input. The team has a friendly atmosphere and strong work ethic.We are seeking a RMN who is looking to be part of future healthcare and integrated working for the older adult population in their locality and supporting them to live independently.Main duties of the jobAs a Senior Community Mental Health Nurse within our Older Adult Service and key member of the team, you will be responsible for managing a defined caseload providing assessment and coordination of care needs, planning, implementation and evaluation of evidence-based nursing care for service users and their carersAs a Senior member of the team, you will need to attend the MDT meetings and to provide professional advice, including health/mental health promotion and support to members of the service as well as other agencies.You will be required to assist the Clinical Team Lead in their absence as appropriate and as a role model and resource for less experienced members of the team, including providing supervision.You will also need to have the ability to work under pressure and although a key part of the team, on your own initiative.Working for our organisationEPUT are looking for motivated staff who shares our Trust values of Care, Learn and Empower.
