Position: Qualified Social Worker - Adult Community Team
Location: Brent Civic Centre, HA9 0FJ
Pay Rate: 32 per hour
Role Overview:
Join the Adult Community Team in Brent, providing comprehensive assessments, support, and interventions to empower adults to lead independent and fulfilling lives. This role focuses on person-centred care, working collaboratively with individuals, families, and multi-agency professionals to address needs, promote well-being, and ensure compliance with statutory duties.
Key Responsibilities:
Assessments & Reviews: Conduct strength-based assessments under the Care Act 2014, ensuring timely reviews of care plans and interventions.
Case Management: Manage a caseload of complex cases, providing tailored solutions that focus on individual needs and desired outcomes.
Collaboration: Work closely with families, carers, and partner agencies to deliver holistic support.
Safeguarding: Identify risks and respond to safeguarding concerns, carrying out investigations and implementing protective measures where necessary.
Community Integration: Support individuals in accessing resources, services, and opportunities to enhance their independence.
Legislation Compliance: Ensure all work aligns with statutory frameworks, including the Care Act, Mental Capacity Act, and other relevant legislation.
Documentation: Maintain accurate records and reports, contributing to the delivery of high-quality, accountable social work services.
Requirements:
Qualification: Social Work degree and registration with Social Work England.
Experience: Proven expertise in adult social care, community-based support, and safeguarding practices.
Knowledge: Strong understanding of the Care Act, Mental Capacity Act, and other relevant legislation.
Skills: Effective communication, problem-solving, and case management capabilities, with a focus on strength-based, person-centred approaches.
Flexibility: Ability to adapt to a hybrid working model, with office-based and community responsibilities.
Working Arrangements:
Hybrid Role: Combination of office-based, community, and remote working.
